Update: New schedule II prescription forms required by June 1

The deadline to begin using new scheduled II prescription forms has been extended to June 1, 2019.

Texas physicians will be required to use new schedule II prescription forms beginning June 1, 2019. The new forms are now available from the Texas State Board of Pharmacy (TSBP). After June 1, official schedule II prescription forms that were issued before September 1, 2018 will no longer be valid.

To order new prescription pads, please visit the TSBP website. It may take up to 30 days to ship prescription forms once they’re ordered, so please plan accordingly.

The TSBP is issuing new forms in response to security issues with the current forms. New forms will include these security features:

  • Control number — a unique number for each prescription form.
  • Pantograph — produces the word “VOID” on the form if someone attempts to copy or scan the prescription.
  • Thermochromic ink — the thermochromic feature will be on the front of the form as a red Rx.
  • Watermarks — the seal of the State of Texas appears in a watermark on the front of the prescription, along with a watermark within the body of the paper.
